1. Make a list of all creditors. Include the
account number, date incurred, balance owed. name, address (including
zip) of the creditor. We will not have you input this information
on the questionnaire. If you are familiar with Excel, you may
enter the info in an Excel document and upload to us on the
questionnaire.
2. Gather all paystubs. Scan and save them to a
location you can remember before starting the form so you can upload
them from the form. I f you do not have a scanner once we assign
you an account number you can fax them to us. If you have more
than 2 paystubs for each person you will have to fax the additional
paystubs to us. There is an upload option for two pay stubs each.
However, if you and your spouse only have four stubs between you (3
might be for you and 1 one for your spouse or you are not filing joint
you can upload all paystubs even if it does not belong to your spouse.
The maximum upload is four.
3. You will need a credit or a debit card to make your
payment. If payment is made by e-check you will have to wait 4
weeks before your documents will be ready for you to print. You
can pay directly from your PayPal account or you can pay using your
credit or debit card via the PayPal site even if you do not have an
account.
